EMT question, really worried?

You should probably start with your instructor, as they know who and how that letter was sent to you. If your instructor is unable to tell you, then you can try your state EMS office. Since the letter came from them, they should be able to just send you a new one. Things like this are usually not a big deal, as this stuff happens all the time.
